Geriatric is medical care that is centered around the older patient and treating their acute and chronic diseases. It can also include a wide array of health-related services for the older adults, including disease care management, specialized medical care, independence, and medication management. Since aging is inevitable and the healthcare needs of the elderly are not the same as the needs for younger patients, geriatric care plays an important role in optimizing both the health can quality of life for older adults. Maintaining the elderly patient’s care in helping them manage a range of conditions and diseases as they age. This can include balance issues, dementia, incontinence, osteoporosis, and many more medical issues. Working as a team with referring to specialists can be incorporated in the elderly patient’s care. The care model is emphasized around the patient, their family, pharmacists, therapists, and other disciplines to provide patients with a holistic approach to health care.
